The Koss Family Foundation is dedicated to individuals and families of Wisconsin living with a cancer diagnosis by providing non-medical financial support so the focus can be on recovery and maintaining quality of life.
The foundation will provide non-medical support for housing, utilities, transportation, and nutritional needs upon review of application and supporting documents. Funds available per household total up to a $1,000 one time grant.
Please note all applications are reviewed on a quarterly basis by month received.
January-March will be reviewed and grants sent out in April.
April-June will be reviewed and grants sent out in July
July-September will be reviewed and grants sent out in October
October-December will be reviwed and grants sent out in January
Patient must currently be undergoing treatment for Stage II or greater, undergoing hospice care, or recently deceased within the past year. If the criteria are not met, the application may require further documentation and review.
Potential clients will be residents of Wisconsin, regardless of where they receive treatment. Residents must provide one (1) proof of address: phone bill, utility bill, bank statements, insurance cards, etc. Please note all applications are reviewed on a quarterly basis as noted above.

Copy of bill(s)-if not received application will be returned
Foundation agreement signed
HIPPA form completed
